Oxford Swimming and Diving Team

I agree to pay for my family's team fees no later than May 30th.

I agree to fulfill my family's volunteer work requirement.

Each OSDT individual or family is required by the city and league to buy a pool pass. This cost covers the pool rental, lifeguards, liability, etc.
Swimmers and Divers may not participate after May 30th unless a pass has been purchased.

I agree to purchase a family or individual Oxford Aquatic Center Pool pass. Must be purchased separately at the Oxford Park and Rec Center Office no later than May 30th.

We are counting on swimmers, divers and their families to do their parts to ensure the season runs smoothly. Please help us focus on positive behaviors this season! We want our coaches to focus on coaching. We want practices and meets to run smoothly.

Seahorse behavior expectations:

1. Swimmers/divers will follow all pool safety rules.
2. Swimmers/divers will listen to coaches’ directions and feedback.
3. Swimmers/divers will show respect to coaches and fellow swimmers.
4. Swimmers/divers will practice good sportsmanship.

1st Offense – A warning will be issued to the swimmer/diver.

2nd Offense – The swimmer/diver will be asked to sit out for the rest of practice and the swimmer’s parents will be notified.

3rd Offense – The swimmer/diver will be asked to remain at home for the next two days. The swimmer’s/diver’s parents will be notified.

4th Offense – A meeting will be held with the swimmer/diver, parent and a board member to discuss the next steps. This may result in the swimmer/diver being removed from OSDT for the remainder of the season with no refund.

Parents, please review the above policy with your swimmer/diver.  The policy will be included with your email confirmation.

I, the undersigned participant or as the parent or guardian with legal responsibility for this participant, acknowledge, agree and understand that:

1. Voluntarily and of my own free will, elect to participate in swimming and/or diving with the Oxford Swim & Dive Team. Furthermore, I agree that I am in good health and proper physical condition to participate in swimming and/or diving.

2. I understand that there are certain risks and hazards involved in participating in swimming and/or diving that may result in injury or death to me or other participants including, but not limited to those hazards associated with swimming/diving conditions, equipment, and other participants.

3. I understand that the very nature of swimming/diving is hazardous and risky, including, but not limited to, the acts of swimming, running, jumping, stretching, diving, and collisions with other participants and with stationary objects, all of which can cause serious injury or death to me and to other participants.

4. I agree that in the event of an injury, I authorize the Oxford Swim and Dive Team to secure first aid and call the Police Department, who will then determine whether or not the child should be taken to the hospital.

Further, I, the undersigned participant or as the parent or guardian with legal responsibility for this participant, agree that in consideration for the right to swim/dive and in consideration for permission to swim/dive:

1. I voluntarily elect to accept and assume all risks of injury incurred or suffered by me (a) while practicing or swimming/diving at a meet as a member of the team so designated, (b) while serving in a non-swimming/diving capacity as a team member during practice or meets by other teams or by other participants on my team, and (c) while on or upon the premises of any and all of the pools arranged for by my team or league for practice or meets.

2. I hereby release, discharge and agree not to sue the Oxford Swim & Dive Team or the City of Oxford, or their owners, officers, agents, servants, associations, employees, or any person or entity connected with for any claim, damages, costs or cause of action which I have or may in the future have as a result of injuries or damages sustained or incurred by me from whatever cause including but not limited to the negligence, breach of contract or wrongful conduct of the parties hereby released.

3. I agree that pictures taken during program hours may be used for promotional purposes and that I give my permission to Oxford Swim and Dive Team to use any images of the participant without compensation.

I ACKNOWLEDGE THAT I HAVE READ AND THAT I UNDERSTAND EACH AND EVERY ONE OF THE PROVISIONS IN THIS WAIVER, RELEASE OF LIABILITY AND INDEMNIFICATION AGREEMENT AND AGREE TO ABIDE BY THEM.
